This pension form was one of several filled out by Zephaniah Phillips. He served in Company F, 56th Regiment, Illinois Volunteers during the Civil War. He also served in other units. This form asks for information about his marriage, his wife and his children.

He is married to Mary E. Phillips, who was Mary E. Pierce. They were married in Harrisburg, Illinois, Oct. 2d, 1862 by william B. Brunner, Minister of the M.E. [Methodist Episcopal] Church. The marriage was recorded in Harrisburg, Saline County, Illinois.

He lists his children and their birth dates: Zephaniah Phillips Jr., born Nov. 23d, 1868; Anna R. Phillips, Aug. 6th, 1866; Clarence Eugene Phillips, July 6th, 1875; and Julia Jennetie Phillips, Sept. 25th, 1870. All living and all married. Anna, the second child, is usually recorded as Anna Clara, but this shows an R. as the middle initial.

This form comes from Zephaniah's pension application files and military records in the National Archives.
